Requirements

  • Bachelor’s degree in social work, human services, public health, or similar are required.
  • Current Basic Life Support (BLS) is required, within 30 days of hire.

  • Ability to maintain appropriate clinical privileges.

Nice To Haves

  • Minimum of two years’ experience in behavioral health services preferred.
  • Experience in behavioral health, integrated care or an FQHC health center is preferred.
  • Bilingual abilities are preferred but not required.

Responsibilities

  • Assists patients with severe and persistent behavioral health concerns to ensure successful treatment and goal management.
  • Assists with grant implementation by coordinating activities, monitoring progress, and ensuring compliance.
  • Collaborates with the medical teams to assist with patient-centered care, promoting the integration of primary and behavioral health care.
  • Provides behavioral interventions and follow-up care as directed by medical and behavioral providers.
  • Maintains current knowledge of community resources and connects patients across all sites to appropriate referrals and support services.
  • Supports patients involved in Medication Assisted Treatment (MAT) program.
  • Educates patients and their families, along with Hunter Health staff on the prevention and treatment of chronic conditions and behavioral health conditions.
  • Adjusts communication with patients, depending on health literacy levels, to ensure understanding of health care plans or concerns.
  • Responds to organizational emergency codes and other crisis situations to evaluate and intervene following crisis interventions procedures.
  • Supports the management of disruptive patient behaviors.
  • Maintains timely records in electronic health record (EHR) and other required paperwork.
  • Provides administrative support for the Integrated Care team.
  • Provides services in person or via telehealth to support patient access.
  • Travels when necessary to meet operational needs.
  • Performs all other duties as assigned.
