About The Position

As a Behavioral Health Clinician (BHC), you will work in a consultative and collaborative role as an integral member of the primary care team, providing behavioral health interventions that support the overall health and well-being of patients. In this role, you will help address a broad spectrum of behavioral health needs within the primary care population by identifying concerns early and providing problem-focused, evidence-based interventions that promote wellness and relieve symptoms. You will assess, manage, and triage psychological and social concerns related to both physical and mental health conditions, partnering with patients and the care team to deliver integrated, patient-centered care. Requirements

• Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington.
• Minimum two years of clinical treatment experience with patients and working as part of a multidisciplinary care team.
• Licensed in one of the following within applicable state: Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT).

Responsibilities

• Deliver evidence-based interventions targeted to improve overall health for patients with a variety of emotional and behavioral problems.
• Meet with patients individually for an assessment and provide recommendations to the primary care team to enhance the patient's care plan.
• Provide on-going skills training and coaching to improve health and functioning for patients over several subsequent sessions when indicated.
• Support helping patients, who require services not provided in the LMG clinic, to make a smooth transition to care with community partners.
• Diagnose and treat mental health and substance use disorders for the identified clinic population.
• Deliver evidenced based therapies such as cognitive behavioral therapy, acceptance and commitment therapy, short term psychodynamic psychotherapy, dialectal behavioral therapy, trauma focused therapy and solution focused interventions.
• Collaborate and advocate for patients’ behavioral and physical health needs with the referring provider.
• Engage patients in health behavior change through brief interventions and health/psychoeducation.
• Coordinate with referral resources and other clinic providers and, where appropriate, community partners to facilitate referral to appropriate specialty treatment.
• Conduct diagnostic assessment of emotional, cognitive, behavioral and substance use disorders for population(s) served in clinical settings.
• Assess cognitive and psychological problems associated with and/or exacerbating medical conditions.
• Deliver trauma informed care.
• Provide individual psycho/social treatment and conduct groups.
• Treat patients with acute and chronic physical health conditions.
