About The Position

Cornerstone Family Healthcare is recruiting a full-time Behavioral Health Counselor for its Behavioral Health team in Port Jervis, NY. The role supports patients and families through mental health and substance use challenges and works collaboratively with providers and treatment programs to deliver integrated care. Cornerstone is a non-profit Federally Qualified Health Center focused on providing comprehensive, high-quality primary and preventive care with dignity and respect, regardless of patients’ ability to pay.

The counselor conducts diagnostic evaluations and psychosocial assessments, provides individual and group psychotherapy, develops and monitors treatment plans, maintains patient documentation, and trains medical staff on mental health and substance use topics. The position includes rotating virtual on-call coverage, one weekly late shift, and participation in meetings and quality improvement activities. Compensation is $65,000–$82,000 annually based on credentials, with sign-on and productivity bonuses.

Requirements

• Master’s degree in Social Work, Psychology, Counseling, or Nursing.
• Active New York State license as an LMSW, LCSW, or LMHC.
• Compassion, teamwork, and a commitment to serving underserved communities.

Nice To Haves

• Minimum three years of post-graduate experience preferred.
• Experience with children and adolescents strongly preferred.

Responsibilities

• Provide diagnostic evaluations and psychosocial assessments for patients with mental health or substance use challenges.
• Deliver individual and group psychotherapy to a diverse caseload, including patients with HIV/AIDS, chemical dependency, and other concerns.
• Collaborate with patients, families, and specialized treatment programs to develop treatment plans and monitor progress.
• Work with other department directors to ensure integrated care.
• Maintain timely and accurate documentation in patient charts.
• Provide in-service training to medical staff on mental health and substance use topics.
• Rotate in a virtual on-call schedule with other counselors.
• Cover one late shift weekly, scheduled Monday through Wednesday, with possible hours of 10–6, 11–7, or 12–8.
• Participate in staff and committee meetings and contribute to continuous quality improvement initiatives.
• Serve as a mandated reporter in suspected child abuse cases.
• Perform other duties as assigned.
