The position allocated to this class is responsible for working with youth and families needing brief intervention and referral services for behavioral health and community resources. Incumbents screen, assess, facilitate and perform case management and care coordination services with state and community partners. Duties are performed with considerable independence in both the office and field and is evaluated on the attainment of the program goals and objectives. Juvenile Court Services provides re-entry and diversion services to youth as part of Washington State Health Care Authority (HCA)’s Reentry Initiative. These services focus on ensuring system-involved youth have access to essential health care resources and coordinated supports that promote stability and successful reentry into their communities. Work will be primarily performed within a 24-hour maximum security juvenile detention facility.
