About The Position

Banner Health is seeking a Behavioral Health Case Manager for its Behavioral Health Hospital in Scottsdale, Arizona. This role involves providing comprehensive care coordination for patients, assessing their plan of care, and developing, implementing, monitoring, and documenting resource utilization and patient progress. The case manager will facilitate options and services to meet patient healthcare needs, with the intensity of care coordination being situational and appropriate based on patient need and payer requirements. The position is accountable for the quality of clinical services delivered and for identifying and resolving barriers to effective patient care. The role may involve floating between adult and adolescent units, as well as Intensive Outpatient Programs (IOP). Requirements

Master’s degree in social work or related field or equivalent experience.
Proficiency level typically achieved with 5 years clinical experience.
Licensure in at least one of the following categories as required by state law: social work, professional counseling, or marriage and family.
Certification requires two years post graduate experience in a mental health setting or equivalent combination of education and documented supervised clinical experience.
Positions providing direct services to substance abuse program patients must also possess board licensure in addictions as required by state.
Working knowledge of care management, hospital and community resources.
Demonstrated critical thinking skills, problem-solving abilities, effective communication skills, and time management skills.
Demonstrated ability to work effectively in an interdisciplinary team format.
Must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of employment (for specific units/roles).
Must sign an Arizona Criminal History Affidavit upon hire (for specific units/roles).
Must possess a Colorado Fingerprint Clearance Card at the time of hire and maintain the card for the duration of employment (for specific units/roles).
Must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain the card for the duration of employment (for specific Banner Staffing roles).

Responsibilities

Manages individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
Provides assessment, direct psychoeducation, and direct psychotherapy to individuals, families, and groups.
Coordinates all aspects of patient care from admission to discharge, interacting with the physician and other members of the clinical treatment team to provide quality patient care.
Acts in a leadership function with process improvement activities for populations of patients to achieve optimal clinical, financial, operational, and satisfaction outcomes.
Evaluates the medical necessity and appropriateness of care, optimizing patient outcomes.
Establishes and promotes a collaborative relationship with physicians, payers, and other members of the health care team.
Collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.
Acts in a leadership function to collaboratively develop and manage the interdisciplinary patient discharge plan.
Effectively communicates the plan across the continuum of care.
Educates internal members of the health care team on case management and managed care concepts.
Facilitates integration of concepts into daily practice.
