About The Position

The Behavioral Health Case Manager MS - Outpatient works with the behavioral health multidisciplinary team to provide care for the patients admitted to inpatient behavioral health or mental health, addictions and child, adolescent day treatment programs. The Behavioral Health Case Manager MS would help with dispositional planning, care planning and group psychoeducation. Performs other duties as assigned.

Requirements

• Master's Degree in a mental health or behavioral health field
• Experience or field study experience in mental health site.
• Exceptional communication skills

Nice To Haves

• Working toward licensure is preferred.
• TACT certified within 90 days after hire.
• Approved CPR/BLS preferred

Responsibilities

• Consults and coordinates care of patient with the physician/psychiatrist, RN, social worker/therapist and other agencies.
• Assesses treatment planning and progress.
• Prepares treatment plan and goals for patients.
• Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy.
• Provides psychoeducational groups.
• Arranges for disposition and follow up recommendations.
