This position provides Case Management services to a caseload of approximately 25 Participants, applying the Housing First approach to serving individuals with housing and other service needs. The role involves active participation in The Louisiana Balance of State Continuum of Care (LA BOSCOC), submitting routine reports and outcome data, and utilizing the HMIS system. Responsibilities include conducting assessments, completing legal documentation, and developing Individualized Service Plans (ISP). The Case Manager will also plan, schedule, and coordinate meetings with Participants and other providers, research and link participants to appropriate services, and collaborate with other providers and landlords to ensure service provision. Regular meetings with a supervisor and active participation in staff meetings and required training are also key components of this role.
