About The Position

The Spirit of Healing Behavioral Health Care Coordinator works independently and collaboratively with a team of health professionals to provide members with opportunities to improve self-management abilities, health outcomes, and satisfaction with health care in a culturally specific manner. This position works collaboratively with American Indian or Alaskan Native members who may have complex medical/psychosocial/ addiction issues and their families/caregivers as well as with members of multidisciplinary teams (MDT) to provide care coordination and mental health assessments as needed. Occasional evening and weekend hours required.

Requirements

  • Bachelor’s degree or higher in behavioral health, social services, psychology, or a related field.
  • Minimum 3 years of experience in behavioral health, care management, care.
  • Active QMHA-I or QMHP-C through MHACBO required
  • Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
  • Ability to operate a motorized vehicle.
  • Demonstrated word processing and computer skills, including ability to learn new systems/databases.
  • Minimum 2 years of electronic health record experience.
  • Ability to maintain professional boundaries.
  • Must pass a pre-employment or for-cause drug tests.
  • Must pass criminal background and DMV checks.

Nice To Haves

  • Active Oregon license (LCSW or LPC) or ability to obtain by start date
  • Experience with utilization of motivational interviewing and trauma informed care.
  • Specialized training in Evidence Based Practices providing behavioral changes.
  • Experience working with diverse populations that have experienced obstacles and barriers related to marginalization.
  • Experience working with Tribal communities and intergenerational trauma.
  • Knowledge of or willingness to learn gender-affirming care needs, available resources, and the transgender experience.
  • Experience working within the Native American/Alaska Native community.

Responsibilities

  • Actively collaborate and coordinate Care Coordination for Spirit of Healing Members/NARA NW patients utilizing a team approach.
  • Serve as the primary behavioral health care coordination for members with severe and persistent mental illness, complex psychiatric presentations, or recent behavioral health crises (including suicide attempts), including transitions of care, and linkage to appropriate treatment and community supports.
  • Administer mental health assessments and treatment plans as needed to streamline member into services.
  • Help coordinate patient transportation and accompaniment as needed.
  • Responsible for scheduling patients discharging the hospital or inpatient settings
  • Work independently to perform timely multidisciplinary case management assessment of members in multiple settings (e.g., home, hospital, clinic, etc.), by evaluating all relevant information to determine needs in health care, social supports and access to behavioral health services.
  • Provide timely updates regarding the plan of care that includes self-management as well as member specific goals.
  • Make independent decisions regarding client care and care management.
  • Complete appropriate documentation in a timely manner.
  • Meet with members to identify and reduce or eliminate barriers to care (e.g., social, cultural, linguistic, education), and set effective goals with members.
  • Collaborate with medical and auxiliary providers in the community to develop health care plans/goals based on identified needs.
  • Up to 80% of the workload will be performed in the community
