About The Position

Professional Care Medical Practice P.C. is currently looking for a Licensed Mental Health Counselor (LMHC) or Licensed Social Worker (LMSW or LCSW) to work as a Behavioral Health Assistant Regional Manager throughout correctional facilities around the Capital Region in New York State. This is a full-time, 40 hour per week position, which includes travel to the assigned region. This includes the following correctional facilities: Saratoga County, Schenectady County, Fulton County, Montgomery County, Warren County, etc.

Requirements

  • Doctorate or Master’s degree in Social Work, Psychology, or other related Human Services degree with a concentration in mental health course work, training and/or experience required.
  • Must be licensed as a LMHC, LMSW, or LCSW in the state of New York.
  • Experience in health care delivery system required.
  • Previous experience in a supervisory role required.
  • Must hold a valid driver’s license & have reliable transportation.

Responsibilities

  • Serve as the designated behavioral health authority whose responsibility is administratively overseeing the health care delivery system within the assigned region.
  • Manage and evaluate the medical programs and staff supervision in the facilities within the assigned region, in accordance with Company policies, procedures, and applicable standards.
  • Engage patients in evidenced based interventions with the goal of stabilizing mental health and substance abuse related symptoms.
  • Actively assist with the detection and prevention of patient suicide.
  • Document all patient encounters in a timely manner utilizing the SOAPE note format.
  • Document response to interventions and modify treatment plans as needed.
  • Assist in the coordination of the admission and placement of patients in forensic and community behavioral health facilities.
  • Monitor clinical encounters and documentation of subordinate staff. Provide clinical consultation and feedback as needed.
  • Monitor subordinate staff for compliance with policies and procedures.
  • Coordinate with the Director of Behavioral Health to facilitate the clinical development and monitoring of subordinate staff.
  • Serve as the behavioral health liaison between the medical unit, facility staff, and corporate staff.
  • Facilitate/participate in interdisciplinary team meetings and/or case consultations with staff across disciplines.
  • Serve as a member of the Quality Assurance Committee and sub-committees, as needed. Assist in the establishment of quality assurance standards and audit criteria
  • Perform all other duties as assigned or delegated by management
