About The Position

The Basic Needs Case Manager attends to the individuals and families in need of Basic Needs case management services, including Homelessness Prevention and assistance to attain public benefits. The Case Manager meets with clients on a regular basis to assess clients’ needs, develop and implement stabilization plans, advocate for the client, and help them organize their monthly household budget. The Case Manager assists in the recruitment and supervision of volunteers and interns for the program and provides support to the Basic Needs food pantry services and reception area as needed.

Requirements

  • A Bachelor degree in Human Services/ Social Work.
  • Two years of experience working as a case manager in a similar environment.
  • Strong interpersonal and communication skills with demonstrated ability to express patience and compassion to all individuals in crisis or under personal stress.
  • Detail oriented with strong organization skills.
  • Ability to work independently and as part of a team.
  • Patience and kindness to work with the public and individuals in need of support.
  • Knowledge of Microsoft Office (i.e. Word, Excel, Adobe, etc).

Nice To Haves

  • Bilingual in English and Portuguese preferred.
  • A valid driver’s license and reliable vehicle preferred.

Responsibilities

  • Attend to individuals and families seeking Basic Needs case management services.
  • Coordinate and complete client prescreening, intake and case note process, and provide case management to the qualifying individuals and families.
  • Oversee, develop and implement individualized service and stabilization plans that are based on the clients’ household needs, risk factors, goals, strengths, and available resources.
  • Assist clients in developing and maintaining supportive networks, and identify community resources for the households. Provides referrals as needed.
  • Conduct budgeting exercises with clients to help them seek self-sustainability, per service requirements.
  • Coordinate homelessness prevention assistance and housing search or advocacy, due to eviction, client safety, rental burden, health issues, and other causes of need.
  • Submit financial assistance requests to the Basic Needs Program Supervisor or administrative office, as funds become available. Assure cases compliance with all applicable regulations and guidelines for financial assistance.
  • Maintain the Basic Needs computer databases and client records for the site and prepare required service reports.
  • Maintain current and help build Catholic Charities Basic Needs collaborations and partnerships within the county served.
  • Recruit, train, and supervise volunteers and interns for the case management services.
  • Participate in building and agency wide Basic Needs meetings.
  • Assist with the front desk operation as needed, by providing phone and other coverage and completing clerical tasks.
  • Local travel as needed. Must have reliable transportation to travel between Agency locations and for attendance at meetings and trainings as required.
  • Work is performed in an office environment with the use of office equipment. Sitting, standing and meeting/talking with clients and business associates for prolonged periods is required.
