About The Position

Responsible for supervision, training, and monitoring workloads of four full-time aging program assessors. Supervisor oversees work by monitoring completion dates for Functional Eligibility Determinations (FEDs) and OBRA assessments to ensure timeframes are met and by making joint supervisory visits as needed. This employee supervises three workers, maintaining contact daily. Conducts formal supervisory sessions with each worker at least monthly. Team meetings are also held monthly.

Requirements

  • Two years of professional experience in public or private social work and a bachelor's degree with major coursework in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR An equivalent combination of experience and education, which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.

  • Familiarity with knowledge of aging related programs, regulations and consumer population.

Responsibilities

  • Screening, assigning, and monitoring referrals in PIA system as well as in-house and community referrals.
  • Sending OBRA reviews to appropriate program office.
  • Completing priority desk reviews in PIA system on a backup basis.
  • Providing ongoing supervision and training to four aging program assessors in completion of least restrictive and appropriate levels of care, for adults age 18 years of age or older who are seeking admission to a nursing home, an approved/licensed personal care home or domiciliary care home or for Home and Community Based Services and who seek public funding.
  • Monitoring staff workload to insure timeframe completion.
  • Primary liaison with Lancaster County Assistance Office regarding status of levels of care and updates status of referrals weekly.
  • Providing oversight and management of the Personal Care and Dom Care recertification data base on a backup basis.
  • Establishing positive relationships with hospitals, nursing facilities, physicians, etc. to facilitate the assessment process.
  • Completing reports on assessment and supervisory activity, monthly.
  • Serving as primary liaison for inter county PAA transfers.
  • Participating in Aging Well Regional Meetings to review PAA procedures and updates and establishing positive relationships with surrounding AAA County supervisors.
  • Following HIPPA standard of confidentiality and privacy of consumer information.
  • Adherence to county and agency policies and procedures.
