About The Position

The Associate, Population Health supports execution of market value-based care initiatives and helps implement population health management capabilities across a network of primary care clinics. Under the guidance of the Manager of Value Based Care Operations, the associate works with practice staff and operations teammates to improve patient outcomes, reduce the cost of care, and enhance patient and clinician experiences. The role monitors and improves daily operational processes and facilitates engagement between patients and practice staff.

Requirements

Hands-on experience with value-based care initiatives and/or positively influencing quality outcomes.
Bachelor’s degree in healthcare or a relevant field.
Experience in the healthcare industry.
Experience with population management and value-based care operations, including Medicare, Medicaid, and commercial lines of business.
Relationship management skills.
Excellent written and verbal communication skills.
A consistent record of working with others to influence desired outcomes.
Experience working with electronic medical records.
Working knowledge of primary care clinic daily operational workflows.
Proficiency with Microsoft Excel, PowerPoint, and Word.
Ability to learn, take initiative, and be resourceful.
Demonstrated ability to execute autonomously and with teams, with personal accountability for results.
High emotional intelligence and the ability to build relevant relationships and navigate diverse partner groups.
Ability to balance critical thinking with hands-on execution.
Excellent presentation, interpersonal, and relationship-building skills.
Knowledge of how to validate data directly tied to population health and value-based care.
Experience driving change.

Responsibilities

Collaborate with value-based care operations team members to develop strategic operational solutions that improve quality while reducing care costs.
Assist the Manager of Value Based Care Operations in coordinating gap-closure campaigns and other market-driven initiatives.
Support outreach efforts for annual wellness visits.
Build working relationships with front office staff, clinical leads, and medical assistants.
Support successful practice performance under value-based contracts.
Create performance reporting using available data and system dashboards to help drive strategy.
Perform other duties as assigned by the Manager.
