Optum NY/NJ is seeking an Associate Patient Care Coordinator to join their team in Carmel, NY. Optum is a clinician-led care organization focused on improving healthcare. As a member of the Optum Care Delivery team, you will contribute to making healthcare better for everyone, supported by the resources of a global organization. The Associate Patient Care Coordinator is responsible for completing set processes and protocols, working cooperatively with the care team to support the organization's vision and mission, deliver excellent customer service, and adhere to Lean processes. This role supports the team in meeting financial, clinical, and service goals. The schedule is Monday through Friday, a 40-hour work week, with hours determined by the supervisor between 8:00 am and 5:30 pm. The location is 670 Stoneleigh Ave. Second Floor, Carmel, NY 10512.
