Associate Patient Care Coordinator
About The Position
Optum NY/NJ is seeking an Associate Patient Care Coordinator to join their team in Hopewell Junction, NY. Optum is a clinician-led care organization focused on improving healthcare delivery. The Optum Hopewell Junction team is looking for a friendly, patient-focused, and detail-oriented Associate Patient Care Coordinator. This role is responsible for completing set processes and protocols, working cooperatively with the care team to support the organization's vision and mission, delivering excellent customer service, adhering to Lean processes, and supporting team goals in finance, clinical outcomes, and service. Schedule: Monday through Friday, 40-hour work week, with hours determined by the manager between 7:30 am and 7:00 pm. Locations: 10 Cranberry Drive, Hopewell Junction, NY 12533 and 30 Columbia Street, Lower Level (ground), Poughkeepsie, NY 12601.
Requirements
- High School Diploma or GED
- 1+ years of computer proficiency experience (multi-task through multiple applications including Microsoft Outlook, Excel, and Word)
- 1+ years of customer service or Healthcare related experience
- Ability to exhibit excellent interpersonal skills
- Ability to travel 25% to both offices as needed and have reliable transportation.
Nice To Haves
- Experience working with an electronic health record (EPIC)
- 1+ years of experience working in medical office position performing duties such as scheduling appointments, checking patients in/out, insurance verification, collecting co-pays, and maintaining medical records
- 1+ years of Knowledge of medical terms
- Bilingual (English\ Spanish)
- Ability to work independently and maintain good judgment and accountability
- Demonstrated ability to work well with health care team
- Strong organizational and time management skills
- Ability to multi-task and prioritize tasks to meet all deadlines

Responsibilities
- Obtain accurate and updated patient information, such as name, address, insurance information
- Assists patients with signing in and completing registration
- Enters and scans patient’s demographic and insurance information into Epic
- Verifies insurance eligibility and benefit information; informs patients of insurance benefits
- Collects co-payments and patient balances according to OPTUM payment guidelines
- Ability to maintain a work queue list- complete any pending actions ie referrals, scheduling appointments
- Confirms and schedule appointments
- Knowledge of medical terminology
- Obtain patient signatures for required documents
- File and maintain medical records
- Answering incoming and outgoing telephone calls promptly and courteously
- Perform referral documentation promptly
- Performs certain follow-up services for patients in a prompt and courteous manner, such as scheduling specialist appointments, scheduling follow-up appointments and obtaining copies lab results or specialist reports
- Adhere to the standards identified via Sparq regarding Optum Employee Policies
- Work cohesively with fellow employees to achieve specific team goals
- Comfortable working in high pace environment
- Participate in planning, coordinating, and implementing care in conjunction with the multidisciplinary team in accordance with the Plan of Care
- Assure the continuity of care through scheduling and tracking systems
- Provide effective communication to patient / family team members and other health care professionals as evidenced by documentation, case conferences, communication notes, and evaluations
- Comply with administrative policies to ensure quality of care
- Demonstrate precision and efficiency in scanning documents and monitoring the fax server, retrieving and / or scanning documents and assigning to the appropriate electronic chart
- Keep abreast on current medical requirements relevant to their position, which includes maintaining patient confidentiality and abiding by all HIPAA and OSHA requirements
- Room patients
- Assist with set up for physical therapy treatment for the provider
- Assist the therapist with medical equipment
- Clean the rooms, ( ie, put away the equipment, organize the space)
- Performs other duties as assigned
