The Optum Hopewell Junction team is looking for a friendly, patient-focused, and detail-oriented Associate Patient Care Coordinator. This role is responsible for completing set processes and protocols, working cooperatively with the care team to support the organization's vision and mission, delivering excellent customer service, adhering to Lean processes, and supporting team goals in finance, clinical outcomes, and service. Schedule: Monday through Friday, 40-hour work week, with hours determined by the manager between 7:30 am and 7:00 pm. Locations: 10 Cranberry Drive, Hopewell Junction, NY 12533 and 30 Columbia Street, Lower Level (ground), Poughkeepsie, NY 12601.
