The Associate Director (AD), Community Partnerships provides strategic, operational, and programmatic oversight of QueensCare and its affiliates’ community health services and external service delivery partnerships. This role is responsible for advancing innovative, community-responsive approaches to care and service coordination, ensuring services and partnerships are adapted to meet evolving community needs and maximize measurable impact. This AD ensures alignment with organizational goals, regulatory requirements, and high-quality standards across all service delivery and community partnerships. The Associate Director, Community Partnerships supports implementation of QueensCare’s strategic priorities by supporting internal and external efforts to improve community health through existing and expanded services and referrals. The Associate Director supervises the Community Health Manager and supports the broader Community Health team, which primarily consists of RNs and CHWs. The Associate Director, Community Partnerships will also be primarily responsible for building and maintaining relationships with community stakeholders across Los Angeles, increasing awareness of and engagement with community health services, and supporting and expanding collaborative partnerships and referral pathways between community health services and external nonprofits and health providers.
