About The Position

Marin Community Clinics, founded in 1972, is a multi-clinic network providing integrated primary care, dental, behavioral, specialty, and referral services. As a Federally Qualified Health Center (FQHC), we serve nearly 40,000 individuals annually in Marin County and have received national awards from HRSA. Our mission is to promote health and wellness through excellent, compassionate care for all. We are seeking a mission-driven Associate Case Manager to join our dynamic, multidisciplinary Complex Care team. This role aims to improve health outcomes for individuals with complex medical, behavioral health, and psychosocial needs, focusing on increasing health equity among vulnerable patients through person-centered, trauma-informed case management. The Associate Case Manager will collaborate with primary care, behavioral health, and other clinical team members to provide patient-centered, outcome-driven care that promotes independence and dignity.

Requirements

  • MA, CHW, Bachelor of SW or equivalent case management experience required.
  • 1-2 years’ experience (and passion for) providing support to individuals with chronic and complex BH needs, complex medical conditions and/or substance-using individuals highly preferable.
  • Experience/familiarity with housing resources/housing navigation in Marin highly preferable.
  • Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required.
  • Training in motivational interviewing highly desired or willingness to learn.
  • Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports and business correspondence in English.
  • Must be able to communicate with patients and partners in Spanish: effectively present information and respond to questions from clients, customers, and the public in Spanish.
  • Must be able to conduct clinical assessment interviews and case management interventions with patients in Spanish.
  • Willing to drive to other MCC Clinic locations in Marin County.
  • Computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.
  • BLS Training Current.
  • Fulfill immunization and fit for duty regulatory requirements.

Responsibilities

  • Support a minimum caseload of 30-40 Complex Care patients.
  • Implement comprehensive patient care plans in partnership with patients.
  • Triage and respond to patient tasks and care needs.
  • Assist patients in completing necessary forms and applications.
  • Work with patients to address social determinants of health (SDOH).
  • Assist patients in connecting with community resources.
  • Assist patients in scheduling and attending medical, behavioral health, and specialist appointments.
  • Provide administrative support, including records retrieval and review, eligibility screening, and goal tracking.
  • Assist patients in coordinating all necessary care.
  • Conduct home/community visits as needed.
  • Consult and coordinate with the interdisciplinary care team, including behavioral health, care navigation MAs, nursing, medical providers, and external partners.
  • Document all patient interactions and services using MCC’s electronic health record and secondary database.
  • Attend internal and external meetings, such as case conferencing and administrative meetings.
  • Attend trainings on topics like health coaching and engagement techniques.
  • Provide timely and accurate reporting of data as requested to MCC CM programs.
  • May travel between clinics or meet patients in the community as needed.
  • Provide frank and considered opinions regarding case management services and quality improvement measures.
  • Provide services to satellite clinics as needed; may be asked to work variable schedules during extended-hour clinics (e.g., 10 am-7 pm on one or more days).
  • Other duties may be assigned.
