About The Position

The Assisted Living Coordinator (LPN) coordinates and addresses the health concerns of Assisted Living residents, including resident assessments, medication and pharmacy management, activities of daily living, and personal care needs. The role collaborates with the Director of Nursing and Assistant Director of Nursing on the day-to-day operation of campus clinical services. The coordinator leads nursing care for assisted living residents and participates in the on-call rotation as designated by the Director of Nursing.

Requirements

• Active, unencumbered LPN license within the state.
• CPR certification.

Responsibilities

• Collaborate with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) on day-to-day clinical services across the campus.
• Ensure each resident’s pre-admission screen is completed before move-in.
• Direct residents’ Lifestyle and Care Needs Assessments to determine initial and ongoing medical and personal care requirements.
• Assist residents with activities of daily living, including bathing, dressing, grooming, and dining, as well as medication administration.
• Lead and assist in developing service plans with residents and families, and ensure the plans are communicated to caregivers.
• Facilitate Assisted Living Resident Meetings and follow up on feedback and concerns in a timely manner.
• Serve as the lead nurse for assisted living residents and participate in the on-call rotation as designated by the DON.
