The Los Angeles County Department of Homeless Services and Housing (HSH) is consolidating its countywide response to homelessness, aiming to increase accountability and transparency, improve care for those experiencing or at risk of homelessness, and streamline collaboration with partners. The Homeless Prevention Unit (HPU) is an initiative involving HSH, California Policy Lab (CPL), and the Department of Mental Health (DMH). HPU uses predictive analytics to identify individuals at high risk of homelessness and offers early interventions. The unit is expanding to serve young adults aged 18-24 with proactive homelessness prevention resources. This role involves significant interaction with various County departments to help clients navigate service systems and connect them to services for housing stability. The ideal candidate will be knowledgeable about programs and services for young people at risk of or experiencing homelessness and be flexible to meet the evolving needs of this population. The Assistant Staff Analyst (Transition Age Youth (TAY) Case Management Program Manager) will act as a supervisory program manager for the HPU TAY program, reporting to the Associate Director. This position will collaborate with HPU and HSH management to develop and implement program structures, processes, and evaluation measures, and will assist in managing daily operations. The TAY Case Management Program Manager will supervise a team of TAY Case Managers who work directly with HPU TAY clients. Responsibilities include designing and implementing policies and procedures to assist clients with complex mental health needs, legal barriers, income barriers, and/or housing insecurity. This role requires building a new team focused on serving young people in Los Angeles County through a client-centered model and developing policies to shape the TAY program within HPU. Experience working with TAY who have experienced or are at risk of homelessness, and a deep understanding of evidence-based practices like Trauma Informed Care and Harm Reduction, are highly desired.
