About The Position

The Segue Residential Transition Program assists with helping individuals transition out of state-run psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.

Requirements

  • A high school diploma or GED and 5 years of experience.
  • An associates degree and 4 years of experience.
  • A bachelor’s degree and 2 years of experience.
  • A driver’s license with 3-points or less and access to an insured vehicle during work hours.
  • Must possess or be able to obtain the CPRP certification by July 1st, 2026.

Responsibilities

  • Oversee program development, coordinate care, and supervise staff with specific focus on the day-to-day operations of the in-reach and service coordinator teams.
  • Overseeing all in-reach processes, ensuring services are authorized and compliant with guidelines.
  • Supervising staff and ensuring effective onboarding, training, and development.
  • Assisting in the development and management of the program budget, policies, procedures, and quality improvement systems.
  • Maintaining compliance with licensing, accrediting, and funding standards, and implementing corrective action plans as needed.
  • Conducting comprehensive clinical assessments, developing treatment plans, and implementing effective interventions for individuals with serious mental illness, including those with co-occurring disorders.
  • Supporting clients involved in the forensic system, ensuring compliance with conditional releases, probation requirements, and collaboration with mental health courts.
  • Developing and implementing strategies to support individuals with mental illness in transitioning successfully from long-term hospital stays to community living.
