About The Position

The Assistant Program Director will work closely with the Director to oversee and support the operation and development of the Care Coordination Program ensuring services are of the highest quality, and in compliance with policies and procedures developed by the Lead Health Home, Community Health Connections, (CHC), Department of Health (DOH), Office of Mental Health (OMH), and Mohawk Opportunities. The role involves assisting with the growth and development of Care Coordination Services as the healthcare landscape changes, assuming supervision and oversight for designated Health Home Care Managers, and carrying a small caseload. The position requires ensuring staff deliver and document services according to program procedures, assisting with staff selection, training, and evaluation, and acting as a resource for staff with consultation as needed to address member-related situations. Responsibilities also include completing member intakes, enrollments, and warm handoffs, representing the agency to other service providers and community groups, and participating in community meetings. The work environment involves multi-level facilities with stairs and client visits to apartments on upper floors. Employees must be able to travel to all levels of the facility and provide transportation to clients using agency vehicles or their personal vehicle. A valid NYS driver's license and approved driver status are required.

Requirements

  • Bachelor’s degree in human services or related field
  • Two years of experience providing services to individuals involved in mental health, substance use disordered, or developmental disabilities systems of care required.
  • Organized and able to effectively communicate with others.
  • Valid and insurable driver’s license.
  • Must have your own vehicle to use for business purposes.

Nice To Haves

  • Supervisory experience preferred.
  • Experience with client database systems and electronic client charting preferred.

Responsibilities

  • Assist with the growth and development of Care Coordination Services as the landscape of healthcare changes.
  • Assume supervision and oversight for designated Health Home Care Managers as well as carry a small caseload.
  • Ensure that designated staff members deliver and document services in a manner consistent with program procedures.
  • Assist with the selection, training, and evaluation of designated staff.
  • Be available as a resource to staff providing consultation as needed to immediately address and problem solve member related situations/issues of concern as they arise.
  • Assume responsibility for completing member intakes, enrollments, and warm handoffs to Health Home Care Managers.
  • Represent the agency to other service providers and community groups and participate in community meetings as required to support the needs of the program.
  • Provide transportation to clients when necessary, using agency vehicles.
  • Use personal vehicle to meet members in the community and attend community meetings.
