About The Position

CareOne at Wilmington is seeking an Assistant Director of Social Services for their facility. This role is responsible for the day-to-day coordination and oversight of all aspects of the Social Service department, adhering to company policies, state and federal guidelines, and the NASW Code of Ethics. The position involves supervising staff, providing direct services, advocacy, counseling, service coordination, education, policy development, and community relations within a long-term care, short-term rehab, and skilled nursing environment.

Requirements

  • Master's Degree in Social Work or a human services field
  • Possess a CSW or LSW within the state of operations
  • 3+ years of Social Work Experience

Responsibilities

  • Plan and direct a comprehensive social service program providing support services for residents and families.
  • Participate in an interdisciplinary team approach to serve the Center population.
  • Provide therapeutic intervention to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities, as well as feelings about institutionalization and separation from family, community, and loss.
  • Maintain written documentation in the resident medical record per company policy.
  • Perform an initial evaluation/assessment of each resident's psychosocial needs.
  • Develop the social service component of the resident's plan of care, identifying problems, goals, and approaches.
  • Participate in and coordinate interdisciplinary care planning conferences.
  • Ensure and document resident and/or family input into the care planning process.
  • Act as a liaison between residents, families, outside agencies, and the Center Administrator to ensure resident rights are maintained.
  • Conduct family meetings as needed (e.g., new admission group meetings, support groups, family and friends council).
  • Function in a management capacity and adhere to all Center policies and procedures.
  • Complete administrative requirements such as forms and reports under the direction of the Administrator.
  • Facilitate discharge planning, including the development of an organized discharge plan for all residents.
  • Ensure the safety of all Center customers to minimize potential for fire and accidents.
  • Ensure the Center adheres to legal, safety, health, fire, and sanitation codes.
  • Coordinate admissions to the facility through generating referrals.
  • Develop and maintain relationships with area referral sources.
  • Verify insurance coverage through computer systems or verbal authorization.
  • Conduct tours of the facility for interested parties.
