The Director of Social Services will abide by and practice according to CareOne Social Service Policies and Procedures Manual, State and Federal guidelines and the National Association of Social Workers & Code of Ethics'. Social Service functions include but are not limited to: supervising social service staff, direct services, advocacy, counseling, service coordination, education, in-servicing, policy and program development and community relations. The role involves acting as a resource to other Social Workers, assisting with recruiting, interviewing, hiring, and orienting new personnel, and establishing standards of practice. The Director will also update and revise the Policy and Provision manual, implement a peer review process, establish communication channels with other Social Workers, and participate in Center-based Quality Improvements. They will submit monthly reports to the RDO and establish partnerships with the Office of the Ombudsman. The position requires planning and directing a comprehensive social service program, participating in an interdisciplinary team approach, and providing therapeutic intervention to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities. Maintaining written documentation in the resident's medical record, performing initial evaluations/assessments of psychosocial needs, and developing the social service component of the resident's plan of care are key responsibilities. The Director will also participate in and coordinate interdisciplinary care planning conferences, ensure resident/family input into the care planning process, and act as a liaison between residents, families, outside agencies, and the Center Administrator. They will conduct family meetings as needed, function in a management capacity, and adhere to all policies and procedures. Administrative duties include completing necessary forms and reports and facilitating discharge planning. The Director must also be concerned with the safety of all Center residents and ensure adherence to legal, safety, health, fire, and sanitation codes.
