About The Position
The Assessor Coordinator Senior will assist the client and family to obtain a high quality of care by jointly developing, implementing and evaluating a plan of care that meets the needs of the client and their family/supporters as the client leaves the hospital to continue with care or to support the client to remain independent in the home setting.
Requirements
- Valid Class 5 driver's license
- Registered/eligible for registration with the appropriate licensing body
- One (1) year experience
- Bachelor of Science in Nursing (BScN)
- Basic - Leadership skills
- Basic - Communication skills
- Basic - Conflict resolution skills
- Basic - Writing skills
- Basic - Organizational skills
- Basic - Problem solving skills
- Basic - Verbal communication skills
- Basic - Interpersonal skills
- Knowledge in quality improvement theory and methodologies
- Ability to work independently
- Ability to work as a member of a multi-disciplinary team
Nice To Haves
- Bachelor of Occupational Therapy is acceptable
- Bachelor of Physical Therapy is acceptable
- Bachelor of Social Work is acceptable
