About The Position

Under the direction of the manager/ clinical supervisor, the Assessor Coordinator - Degree provides full scope case management services in assigned acute care area. Through collaboration with family, care givers, and a multi-disciplinary team, this position provides case management to patients who require discharge planning within acute care. The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluation, termination/discharge. The Assessor Coordinator - Degree liaises with services across the province, collaborating with acute care, rehabilitation, home care, continuing care, mental health, and various government and non government agencies to assess patient needs and coordinate services. Case management is patient-centered, strength-based, flexible, cost-conscious and quality driven. A key function of case management is to advocate for, and coordinate resources based on assessed need.

Requirements

  • Bachelor of Social Work (BSW) or Bachelor of Nursing
  • Valid Class 5 driver's license
  • Current practicing registration with SASW or CRNS
  • Standardized documentation skills which accurately reflects clinical practice

  • Ability to facilitate case/family conferences.

  • Knowledge of the dynamics of abuse and neglect
  • Demonstrated ability to practice core functions of case management
  • Knowledge in client case management and process facilitation
  • Knowledge in the determinants of health and impact on health
  • Knowledge of Health Information Protection Act and other legislation.
