About The Position

The Assessment and Intake Nurse delivers compassionate, culturally competent, trauma-informed nursing care to adult clients in residential (substance use disorder and/or mental health) treatment at the Healing Center. The Assessment and Intake Nurse plays a key role in ensuring medical safety, supporting withdrawal management protocols, administering medications, and working collaboratively with an interdisciplinary team. The Assessment and Intake Nurse serves as a leader in the healing Center’s effort to provide a culturally respectful healing environment, with special emphasis on the needs of Native American clients, particularly those from the four Dakota tribes. The position will often be the first face that patients see upon arrival and will set the tone for the patient’s advancement through the program. In that respect the Assessment and Intake Nurse will begin the Nursing Assessment with patients that arrive during normal business hours unless attending to other duties. The nurse ambassador will assist the Medical Director and Physicians Assistant with clinic operations including appointments, rooming, obtaining vital signs, and any other duty that arises in day-to-day operations. In addition to this, the Assessment and Intake Nurse will assist as needed with scheduling and operations of the TMS Department and will, at times, administer TMS in the absence of the TMS technician.

Requirements

  • Current, unrestricted license as a Registered Nurse (RN) in the State of Minnesota.
  • Current BLS; ACLS preferred.
  • One (1) year of nursing experience in a behavioral health, medical-surgical, or residential care setting.
  • Strong interpersonal and communication skills.
  • Extensive knowledge of Dakota cultural values and healing practices.
  • Proficient with basic computer and electronic documentation skills; experience with NextGen a plus.
  • Ability to work with compassion, discretion, and sound clinical judgment.
  • Must pass a background check and meet tribal credentialing requirements.
  • Must possess a valid driver’s license, pass a motor vehicle records check, and be insurable under the Healing Center’s policy to transport clients.
  • This position is subject to testing for drugs (THC included) and alcohol in accordance with Federal Department of Transportation regulations, as well as the SMSC Drug/THC and Alcohol Policy.

Nice To Haves

  • Experience in withdrawal management or substance use disorder treatment preferred.
  • ACLS certification

Responsibilities

  • Assess, formulate a nursing plan of care, document, and respond to medical needs and collaborate with medical providers and interdisciplinary staff.
  • Assessment and Intake to include: Medical History Review, Physical Examination, Initial Psychiatric and Substance Use Assessments, functional and social assessments, safety and risk assessments.
  • Provide direct nursing care including medication administration, symptom monitoring, vital signs, and withdrawal management according to clinical protocols.
  • Serves as first contact liaison and welcomes all new patients to the program.
  • Monitor and manage withdrawal symptoms and implement provider-ordered medication assisted treatment (MAT) when appropriate.
  • Maintain accurate and timely nursing documentation in the electronic health record (NextGen).
  • Assists the program Psychiatrist and PA with scheduling, rooming, obtaining vitals, and any other duties that arise in relation to the clinic portion of the Healing Center.
  • Support safe medication practices, including inventory, storage, administration, and disposal of controlled substances.
  • Conduct nursing groups and individual sessions to educate clients on medications, health conditions, wellness, and self-care practices.
  • Assist with crisis intervention and emergency response, including coordination with emergency medical services.
  • Communicate effectively with clients, staff, and outside healthcare providers to support continuity of care.
  • Drive clients to offsite medical appointments, community-based services, or other approved events using Healing Center motor pool vehicles.
  • Support culturally respectful care in partnership with cultural advisors and practitioners.
  • Participate in team meetings, case reviews, and quality improvement initiatives.
  • Perform other duties as assigned in support of the Healing Center’s mission and client well-being.
  • Operate the TMS system safely and according to established protocols.
  • Provide patient support before, during, and after TMS sessions, ensuring comfort and safety.
  • Follow all safety protocols related to the operation of TMS equipment
