Position Summary
The Intake Counselor/Assessment Coordinator is responsible for being able to perform all aspects of Central Florida Behavioral Hospital’s intake and admission process for both Inpatient and Outpatient Servives, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual’s mental health status; making patient safety and level of care recommendations to the CFBH physician; identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information in Med Series 4.

Requirements

• Ability to pass a Level 2 Background Investigation.
• Masters Degree from an accredited college or university in social work, mental health counseling
• Must be available to attend the full hospital and departmental orientations: Week 1: M-F 8 AM - 5 PM

Nice To Haves

• Registered Intern, preferred
• Minimum of two (2) years experience in the mental health field preferred.

Responsibilities

• Responds to telephone inquiries
• Knows CFBH programs and is able to answer general program information
• Determines initial clinical and financial eligbility
• Conducts psychosocial evaluation on each assigned admission, gathers data
• Coordinates with the treatment team, family and patients
• Asissts with discharge planning
