About The Position
Carolina Outreach is seeking a compassionate, dedicated, and skilled Psychiatric Nurse (RN) to join our Assertive Community Treatment (ACT) Team. This role involves delivering critical, community-based mental health services to individuals with complex needs. The ACT Nurse will be central to service delivery, partnering with clients, families, and an interdisciplinary team to ensure clients receive necessary care and support. The role involves overseeing the medical and clinical components of treatment, blending clinical expertise with a commitment to person-centered care.
Requirements
- Registered Nurse must be licensed to practice in NC by the Board of Nursing.
- A valid driver’s license and current automobile insurance is also required.
- Each ACT team member must complete DMH-approved ACTT training.
- Must demonstrate competence in providing ACT services prior to service provision.
Responsibilities
- Monitor and oversee all medical/clinical aspects of services delivered.
- Provide medication monitoring and documentation.
- Deliver psychopharmacologic and medical services within your scope of practice.
- Collaborate with ACT team members to monitor clinical status and response to treatment.
- Conduct crisis assessments, symptom management, and intervention planning.
- Provide individual counseling, substance abuse treatment, and psycho-educational support.
- Assist in creating transition plans when clients move in and out of services.
