Requirements

Master’s Degree in health related field
NJ Provisionally Licensed Social Worker, NJ Licensed Associate Counselor, NJ Licensed Associate Marriage and Family Therapist
Valid driver’s license required/ Vehicle required
No more than one moving violation within the past 12 months

Responsibilities

Respond to behavioral-health-related calls for service, including crises, confused/disoriented individuals, welfare checks, or other mental-health-related needs identified by law enforcement.
Provide follow-up support to individuals and families.
Deliver crisis intervention counseling to stabilize psychiatric emergencies.
Provide mental health assessment, evaluation, treatment planning, and referral.
Advocate for individuals to secure needed services and support.
Develop individualized care plans in collaboration with community agencies.
Collaborate with community providers, including psychiatric screening center when clinically appropriate.
Complete all required documentation, reports, assessments, and submissions, including Benchmark.
Coordinate with RWJ Hospital or RWJ PESS staff regarding screenings or transportation for individuals who may meet criteria for psychiatric evaluation.
Maintain individuals’ rights and ensure confidentiality.
Provide appropriate short-term follow-up to ensure linkage, stabilization, and continuity of care.
