Applied AI Engineer (Forward Deployed) [NYC or SF]

Concurrence•New York, NY
•$200,000 - $300,000•Onsite

About The Position

Concurrence builds a platform healthcare organizations use to create clinical AI agents. Its Health World Model unifies health system data into an event-level record of patient care, while agents support work across the patient journey. The company reports that its agents have supported more than 10 million patient encounters. Concurrence is a roughly 35-person company working in person in New York and San Francisco. Applied AI brings the platform into health systems and builds durable production software for each customer. The engineer owns complete customer use cases, from deployment brief through production and beyond, including architecture decisions, timelines, and outcomes. Success is measured by customer results such as increased intake handled, fewer denials, and faster billing close. The role involves building and evaluating agent systems for clinical settings, integrating varied EHRs, legacy systems, and customer data, and turning proven customer-specific patterns into reusable platform capabilities. The engineer also identifies opportunities to replace agent reasoning with deterministic data lookups, raises scope and integration risks early, mentors teammates, and contributes to team practices and documentation. The role is intended for a senior-level engineer with a path to Staff or Tech Lead. The working week includes collaborative in-office sprints early in the week, a protected focus day on Thursday, and Friday for consolidating learnings.

Requirements

  • Experience shipping and owning complex production systems end to end with real users depending on them.
  • Experience building with LLMs and agents in production, or sufficient engineering depth to develop that capability quickly.
  • Rigor in evaluating non-deterministic systems, using evidence rather than a small number of anecdotal runs.
  • Ability to balance near-term costs and long-term effects of technical decisions.
  • Comfort working directly with customers and clinicians to understand workflows.
  • Strong engineering craft, including building systems that expose failures early and remain reliable.

Nice To Haves

  • Healthcare experience, including EHR integrations such as Epic, Oracle Health, or athenahealth; HL7; FHIR; or clinical workflows.
  • Experience with Databricks, voice agents, forward-deployed engineering, or customer engineering.
  • Experience at an early-stage startup.

Responsibilities

  • Own customer use cases end to end, from deployment brief through production and beyond.
  • Make internal architecture decisions, commit to timelines, and deliver outcomes.
  • Build agent systems suitable for clinical settings, using agent and skill hierarchies and rigorous evaluations.
  • Ensure systems clear the company's safety floor and evaluation gate before reaching patients.
  • Integrate EHRs, legacy systems, and customer data with the platform.
  • Turn field-proven patterns into first-class platform capabilities.
  • Move proven work from agent reasoning into deterministic data lookups where appropriate.
  • Raise concerns early when specifications are unvetted, integrations unsupported, or scope changes.
  • Mentor engineers on the use case and help shape how the team builds.
  • Write documentation usable by coding agents and new hires.

Benefits

  • Comprehensive health, dental, and vision insurance.
  • Daily catered lunch and dinner.
  • Mental health support and wellness coaching.
  • Flexible wellness stipend for fitness, therapy, or personal growth.
  • Annual learning budget for courses, books, or conferences.
  • Conference attendance budget for professional development.
