About The Position

Benefis Health System, in Great Falls, Montana, is looking for an advanced practice provider to provide specialized care to older adults 65+ admitted with acute medical conditions following traumatic injury, emphasizing geriatric assessment, addressing functional decline and complex care needs, interdisciplinary coordination and safe transitions of care.

Requirements

  • 5+ years’ experience in inpatient medicine, geriatrics, or acute care with ability to manage complex medical needs in a fast-paced collaborative environment
  • Strong competency in diagnostic reasoning, geriatric syndromes, and ability to have respectful interdisciplinary communication
  • Desire to grow and expand services based on patient needs and program evolution

Nice To Haves

  • Trauma service experience strongly preferred
  • Adult‑Gerontology Acute Care Nurse Practitioner (AGACNP) or Adult‑Gerontology Primary Care NP (AGPCNP) preferred but other degrees will be considered based on experience

Responsibilities

  • Conduct comprehensive geriatric assessments, including functional status, cognitive screening, frailty evaluation, fall risk, and polypharmacy review.
  • Develop and adjust individualized inpatient care plans based on geriatric principles and patient goals
  • Partner with medical teams to manage acute medical conditions common in older adults and optimize management of chronic diseases such as diabetes, hypertension, CKD, dementia, and osteoporosis.
  • Provide delirium prevention, management, and education including non‑pharmacologic strategies and safe medication use
  • Review medications, ensuring careful attention to renal dosing, drug interactions, and deprescribing when appropriate, in coordination with primary care teams and pharmacy
  • Order, interpret, and integrate diagnostic tests (labs, imaging, EKGs) into patient care plans when appropriate
  • Prepare patients and families for safe discharge, including education, follow‑up planning, and coordination with outpatient providers or skilled nursing facilities.
  • Collaborate closely with hospitalists, specialists, nursing, PT/OT, case management, social work, and palliative care.
