About The Position

The Osher Center for Integrative Health at Vanderbilt University Medical Center offers a 12-month postgraduate Integrative and Lifestyle Fellowship. This program is designed to train nurse practitioners and physician assistants in providing compassionate, evidence-based integrative and lifestyle medicine for the prevention and treatment of chronic diseases. Fellows will work within an interdisciplinary team, gaining experience in integrative and lifestyle management of health. They will learn to conduct integrative and lifestyle consultations, facilitate medical group visits, and engage in collaborative care with various health professionals including physical therapists, psychologists, massage therapists, acupuncturists, and yoga/t’ai chi instructors. The fellowship provides training and mentorship from physician and nurse practitioner colleagues in the Department of Physical Medicine and Rehabilitation.

Requirements

  • Adult Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult Gerontology Acute Care Nurse Practitioner (AGACNP/ACNP), Family Nurse Practitioner (FNP), or Physician Assistant (PA)
  • Master's degree in Nursing Advanced Practice
  • Relevant Work Experience
  • Less than 1 year of experience

  • Eligible for Board Certification in Lifestyle Medicine through the American College of Lifestyle Medicine

Responsibilities

  • Performs and documents a complete history and physical examination.
  • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information, and diagnostic procedures.
  • Assesses for risks associated with care, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.
  • Participates in daily huddles or rounds with the interdisciplinary team and collaborates in formulating and evaluating the ongoing plan of care.
  • Collaborates with the interdisciplinary health care team in making accurate diagnoses of acute and complex chronic conditions.
  • Manages diagnostic tests through ordering, interpretation, performance, and supervision.
  • Formulates differential diagnoses by priority and diagnoses complications, identifying appropriate interventions.
  • Identifies expected outcomes from diagnosis and formulates and documents a plan of care to address acute and complex, chronic health care needs.
  • Utilizes evidenced-based practice guidelines or protocols in an individualized, dynamic plan of care.
  • Prescribes diagnostic strategies and therapeutic interventions (pharmacologic, non-pharmacologic, and surgical as needed) to achieve expected outcomes.
  • Manages further diagnostic tests through ordering, performance, and interpretation.
  • May perform advanced procedures consistent with privileges and competency validation.
  • Maintains ongoing communication and collaboration with interdisciplinary health care team members.
  • Facilitates communication with patient, family, and staff to promote continuity of care across the inpatient and outpatient continuum.
  • Documents key components of patient's progress via daily progress note, transfer, and discharge summary and/or clinic note.
  • Provides preceptor/mentoring, education, and instruction of students and nursing staff as needed.
  • Seeks opportunities for active engagement in research and the analysis of evidenced based practice.
  • Attends Grand Rounds, AP Council meetings, faculty and unit meetings, MM & I presentations, and other education opportunities.
  • Maintains continuing education to meet licensure and certification requirements.
  • Actively participates in the required professional development fellowship curriculum.
