About The Position

This is a full-time position at Beth Israel Lahey Health's central location in Boston, MA. The Antimicrobial Stewardship Pharmacist 2 will be an integral part of the growing Antimicrobial Stewardship system program, contributing to the expansion of dedicated services across hospitals in Eastern Massachusetts and Southern New Hampshire. This role is the second tier in a four-level career ladder, reporting to the Manager of Antimicrobial Stewardship. The pharmacist will work under the general supervision of ASP leadership and senior AS pharmacists to assist with daily AS activities, focusing on improving antimicrobial utilization through various processes. This includes evaluating provider order entry, microbiology results, patient data, and alerts for best practices, quality, and safety. The position involves assisting in tracking team interventions, serving as a resource for other specialists and trainees, and participating in the orientation and training of frontline staff. The pharmacist will identify and propose solutions for clinical-operational problems, review standard antimicrobial susceptibility reports, and receive ongoing training and professional development.

Requirements

  • Bachelor's degree in Pharmacy required.
  • License to practice as Pharmacist in Massachusetts or New Hampshire depending upon practice location.
  • Completion of PGY 1 Pharmacy Residency that includes Infectious Diseases training or at least 2-3 years’ work experience in inpatient antimicrobial stewardship.
  • Advanced skills with computer systems required, including web-based applications and Microsoft Office suite applications.

Nice To Haves

  • Doctoral degree in Pharmacy (PharmD) preferred.
  • Certification in Antimicrobial Stewardship from a national accrediting provider- Society of ID Pharmacists, Infectious Diseases Society of America, Society for Healthcare Epidemiology of America.
  • Active Board certification in Infectious Diseases Practice OR eligibility to take the exam and commitment to attain within 1-2 years in ASP2 role.
  • Detail oriented, critical thinker, effective communicator with analytical abilities and strong interpersonal skills.
  • Experience in use of electronic health record, particularly Epic, including antimicrobial stewardship tools and/or other related software preferred.
  • Knowledge and/or experience in related fields such as microbiology, infection prevention, public health and quality improvement.

Responsibilities

  • Practices pharmacy with a focus on antimicrobial management in accordance with all applicable laws, rules, regulations, and established policies and procedures. Maintains licensure and credentials.
  • Reviews ASP1 preliminary recommendations and, under the direction of ASP3-4, prioritizes telehealth or onsite initiatives. Assumes responsibility for daily AS program activities including pre-prescriptive approval reviews, post-prescriptive review and feedback, and clinical decision support alerts reviews.
  • Reviews and verifies anti-infective medication orders considering patient-specific factors such as diagnosis, weight, age, allergies, organ function, laboratory indices, medication duplication, and drug interactions. Clarifies orders with prescribers and provides education. Documents AS interventions and communicates applicable information to subsequent shifts.
  • Reviews ASP1 and trainee assessments of daily pharmacy surveillance tool monitoring. Independently interacts with providers to initiate therapeutic drug monitoring and documents activity in the electronic medical record. Creates reports of compliance and acceptance for interventions.
  • Promotes a culture of patient safety by identifying and recording medication-related events or policy deviations for antimicrobial ordering.
  • Participates in the development, evaluation, and validation of the electronic medical record relative to clinical decision trees, guided decision making, order questions, and antibiotic utilization and resistance submissions.
  • Provides in-service education to healthcare personnel on new formulary additions, treatment guidelines, and best practice alerts. Participates in established AS- or Pharmacy-based proactive programs related to clinical operations and quality assurance. Plans and manages resources and communication during transitions between units, at discharge, or with external healthcare organizations.
  • Demonstrates good judgment by asking for assistance from AS3-4s, AS Director, and Medical Directors as appropriate. Performs additional duties as assigned.
  • Maintains strict adherence to BILH confidentiality guidelines and compliance with HIPAA.
