About The Position

The Infectious Diseases Clinical Pharmacist will support and advance Antimicrobial Stewardship Program (ASP) initiatives across the Spaulding Rehabilitation Network and island hospitals including Martha's Vineyard Hospital and Nantucket Cottage Hospital. The position combines direct clinical support with system-focused stewardship efforts across a network of acute rehabilitation and critical access hospital sites. This pharmacist will collaborate closely with interdisciplinary clinical teams to optimize antimicrobial therapy, support evidence-based prescribing practices, and promote safe and effective use of anti-infective agents across these diverse care settings. Key responsibilities include prospective audit and feedback, interpretation of microbiologic results, and providing patient-centered recommendations for antimicrobial regimen optimization aimed at improving clinical outcomes while minimizing antimicrobial resistance, medication-related toxicities, and other unintended patient harms. Additional responsibilities include development and implementation of clinical guidelines, workflows, as well as support for regulatory compliance, accreditation readiness, and quality improvement initiatives.

Requirements

  • Doctor of Pharmacy Pharmacy required
  • Pharmacist [State License] - Generic - HR Only required
  • experience working as a pharmacist 3-5 years preferred
  • Extensive knowledge and ability in all areas of the department including: IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution.
  • Attention to detail.
  • Demonstrate appropriate knowledge of principles of growth and development over the life span of the neonate, adolescent, adult, and geriatric patient.
  • Possess ability to assess patient data relative to age specific needs and Provide care as described in the department's policies/procedures.
  • Must complete 20 continuing education credits per year to maintain license.

Nice To Haves

  • Completion of a PGY2 residency in Infectious Diseases Pharmacy, or Completion of a PGY1 pharmacy residency plus a minimum of 2 years of Infectious Diseases Pharmacy practice experience or Minimum of 4 years of Infectious Diseases Pharmacy practice experience
  • Board certification in infectious diseases pharmacy (e.g., BCIDP or equivalent)

Responsibilities

  • Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes.
  • Provides direction and guidance to pharmacist and ancillary support including technician staff in following policy and procedure, taking accountability and ownership of job responsibilities, and promotes professional behaviors and attitudes.
  • Provide feedback to the pharmacy managers/practice managers/medical directors for the interim and annual performance appraisal of other pharmacists, pharmacy technicians, pharmacy interns, physicians, medical residents, medical interns and medical assistants when appropriate.
  • Participates in resolving personnel management issues, when requested by the management staff.
  • Operates within the limits of pharmacy policy and procedures.
  • Ensures compliance with target drug programs, formulary management, medication management initiatives, and adverse drug event monitoring programs.
  • Documents and reports clinical interventions, medication errors, adverse drug events.
  • Participates in department quality improvement efforts.
  • Attends and participates in daily medical and multi-disciplinary rounds and participate in specialty rounds as needed and as time allows and collaborate with other healthcare providers when staffing non-rounding services.
