This is a Full Time opportunity for a Clinical Pharmacy Specialist, Antimicrobial Stewardship to join the team at Surrey Memorial Hospital. The role involves providing pharmaceutical care and service excellence to patients, residents, and clients. The Clinical Pharmacy Specialist will consult with healthcare professionals, provide clinical direction on complex cases, participate in patient education and discharge consultations, and assist in developing drug therapy guidelines. The role also includes identifying and initiating clinical drug research, preparing submissions for professional journals, and conducting pharmacological education sessions. Additionally, the specialist will represent the department on multidisciplinary care committees and external events, and ensure pharmacists have current information for prescription validation and safety concern identification. The position reports to the Coordinator, AMS and collaborates with the Coordinator, Clinical Pharmacy Services, playing a major role in planning and implementing the Antimicrobial Stewardship Program (AMS). This includes identifying target areas for antimicrobial drug usage evaluation, preparing reports, developing criteria and standards, implementing changes to the AMS Program, and monitoring drug-utilization data. The role involves developing and implementing AMS policies and procedures, researching best practices, conducting critical reviews of clinical literature, and consulting with clinical experts. The specialist will also conduct antimicrobial drug utilization evaluations, develop communication materials for the AMS Program, and maintain expertise in applicable pharmacy areas through continuing education. The role includes educating staff on clinical literature interpretation, supporting the pharmacy residency program, acting as a clinical pharmacy specialist for the AMS Program, and participating in the development of clinical pathways and therapeutic protocols. Representation on multidisciplinary committees and participation in quality improvement activities are also key aspects of the role. The specialist will maintain records, prepare reports, and perform other related duties.
