About The Position

Under minimal supervision from the Hawaii Keiki (HK) RN Care Coordinator, the HK Allied Health Professional Care Navigator plays a supportive, non-clinical or semi-clinical role focused on helping patients access services, follow care plans, and navigate healthcare systems efficiently. The position is centered on coordination, communication, and removing barriers to care while working under the guidance of the RN Care Coordinator. This position serves in a dual-capacity role by also serving as an EPIC Superuser. This position ensures the efficient operation of the HK program through a combination of technical expertise, coordination, and compliance. In addition, they may also provide general health support for schools that may include assisting with the coordination of school wide screenings with community partners, health and wellness promotion, data collection and disease surveillance, coverage of the health needs of students, data entry and health education as requested. Finally, they may be asked to facilitate virtual and/or in person visits with a HK Nurse Practitioner (APRN). This position is funded by the Rural Health Transformation Grant and supports the pilot sites under this grant.

Requirements

  • High School Diploma or equivalent
  • Certification in a health related field or equivalent job related work experience
  • Current BLS certification and First Aid Certification
  • Experience working in a clinical setting
  • Experience working with front and back-office medical operations
  • Experience using EPIC
  • Experience working with a healthcare provider
  • Excellent verbal, written and interpersonal communication skills
  • Excellent organizational skills
  • Customer service skills
  • Ability to communicate and guide parents/guardians through the consent process
  • Ability to communicate and guide parents/guardians through the insurance process
  • Familiarity with Patient Confidentiality laws (HIPAA/FERPA)
  • Ability to obtain and recognize abnormal vitals signs
  • A strong understanding of clinical and/or billing workflows
  • Excellent problem-solving and analytical skills
  • Skilled in managing multiple priorities simultaneously in a fast-paced environment
  • Exceptional communication and interpersonal skills
  • Ability to work both independently and collaboratively with little supervision
  • Detail-oriented with a commitment to accuracy and quality
