About The Position

Join the Geriatric Outreach Service as an Allied Health Assistant and help improve care and transitions for older people across the Limestone Coast. Working as part of a multidisciplinary team alongside senior allied health and nursing clinicians, you will support older people experiencing dementia, cognitive decline and complex health or functional needs. Under the direction of Allied Health Professionals, you will deliver delegated interventions, support functional independence and wellbeing, and assist with care coordination, appointments and communication with residential aged care facilities. You will also play an important role in supporting safe, coordinated transitions from hospital to residential aged care. This is a Monday to Friday role with no after-hours requirements, with flexibility available around the days of the week worked and hours worked each day.

Requirements

  • Hold a Certificate IV in Allied Health Assistance, or equivalent.
  • Demonstrate an aptitude for working with older people with complex health, functional, cognitive and psychosocial needs, including people from culturally and linguistically diverse backgrounds.
  • Be able to work under general direction and direct or remote supervision from Allied Health Professionals.
  • Bring strong written and verbal communication, interpersonal, time management, organisational and problem-solving skills.
  • Be able to work in a physically demanding environment, including assisting with patient therapy and handling patient-related equipment and devices.
  • A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups is required for this position.
  • A current minimum C Class driver’s licence is required.
  • Immunisation Risk for this position is – Category A.
  • SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.
