Requirements

• Must meet the PA residency requirement.
• Must be able to perform essential job functions.
• A conditional offer of employment will require satisfactory completion of a medical examination and drug screening.
• This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

Responsibilities

• Assist with completing assigned portions of assessments and planning documents to ensure accurate information
• Conduct assigned training activities to help individuals learn daily living skills
• Record and report information to support updates and improvements to each individual’s support plan
• Share observations and insights with team members to support coordinated care
• Help maintain a safe, comfortable, and therapeutic living environment
• Model appropriate behavior and uphold individual rights and dignity
