Requirements

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
• Meet the PA residency requirement.
• Be able to perform essential job functions.
• Satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
• Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

• Gather information about consumers and their situations to identify needs and locate appropriate services
• Provide support to consumers and families to help them adjust to personal or social challenges
• Make home visits and phone calls in alignment with established plans and agency policies
• Complete required assessments and maintain accurate records in SAMS and PIA systems
• Help consumers understand and receive benefits and provide information on relevant laws
• Work with community resources to ensure consumers can access available services
