SNF Advanced Provider (Sandusky, OH)
About The Position
Ennoble Care is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide in-home primary and palliative care to homebound patients in and around Sandusky, Ohio. This is a full-time position. The provider applies medical techniques and principles under physician supervision and cares for patients with chronic diseases as part of a team. The role involves assessing patients, developing diagnoses and treatment plans, coordinating with office staff and clinical operations, and working with patients, families, and caregivers. Compensation is $105,000–$145,000+ depending on experience and other incentives. The position offers autonomy, support from an in-house clinical team, and the opportunity to join a growing leadership team. Requirements
- Board certification and a valid Nurse Practitioner or Physician Assistant certification in the State of Ohio.
- Strong clinical skills and the ability to accurately and confidently diagnose, assess, and treat patients.
- Positive demeanor, teamwork, and strong interpersonal skills.
- Ability to problem-solve confidently.
- 1–2 years of primary care experience with geriatric patients.
Nice To Haves
- Hospice experience is highly preferred but not required.
Responsibilities
- Provide high-quality primary and palliative care to patients in and around Sandusky, Ohio.
- Assess patients through physical examinations and by obtaining, updating, and reviewing medical histories; develop appropriate care plans.
- Communicate effectively with office staff and clinical operations using internal platforms.
- Develop differential diagnoses and treatment plans.
- Build trust and consult with patients, families, and caregivers.
- Order diagnostic testing when medically necessary.
- Prescribe medications and supplies and provide appropriate medically necessary interventions.
