About The Position

Ennoble Care is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide in-home primary and palliative care to homebound patients in the Sandusky, Ohio area. The role applies medical techniques and principles under physician supervision and serves patients with chronic diseases as part of a care team. The position is full-time and offers autonomy in delivering care, support from an in-house clinical team, and opportunities to join the growing leadership team.

Requirements

• Board certified, with a valid Nurse Practitioner or Physician Assistant certification in Ohio.
• Strong clinical skills and the ability to accurately and confidently diagnose, assess, and treat patients.
• Positive demeanor, teamwork orientation, and strong people skills.
• Ability to confidently solve problems.
• One to two years of primary care experience with geriatric patients.

Nice To Haves

• Hospice experience is highly preferred but not required.

Responsibilities

• Provide high-quality primary and palliative care to patients in and around Sandusky, Ohio.
• Assess patients’ health through physical examinations and by obtaining, updating, and reviewing medical histories; develop care plans.
• Communicate effectively with office staff and clinical operations through internal platforms.
• Develop differential diagnoses and treatment plans.
• Build trust and consult with patients, families, and caregivers.
• Order diagnostic testing when medically necessary.
• Prescribe medications and supplies, and provide appropriate medically necessary interventions.
