About The Position

Requirements

Current NP or PA license in the state of Oregon
Graduate degree from an accredited NP or PA program
National certification as an NP or PA
DEA registration (or eligibility)
3+ years of NP/PA experience, primary care experience preferred
Current BLS & ACLS certifications
Strong clinical, communication, and patient care skills
Experience with electronic health records (EHR)
Ability to work independently and collaboratively in a team environment

Responsibilities

Provide comprehensive healthcare services including assessment, diagnosis, treatment, preventive care, and chronic disease management to patients with a wide range of healthcare needs and conditions.
Work collaboratively with physicians and the care team to deliver high-quality, patient-centered care.
Conduct patient assessments and physical examinations.
Diagnose and treat acute and chronic conditions.
Order, interpret, and review diagnostic tests and imaging.
Develop and implement treatment plans.
Prescribe medications in accordance with state regulations.
Provide preventive care, screenings, and health education.
Manage chronic diseases and monitor patient outcomes.
Coordinate referrals and follow-up care.
Maintain accurate and timely medical documentation.
