About The Position

Affinity Health Center is seeking an experienced Advanced Practice Provider (Family Nurse Practitioner or Physician Assistant) to provide compassionate, evidence-based primary care and build ongoing care relationships with patients. The role includes preventative and wellness care, chronic disease management, and acute-episodic care, with a focus on walk-in patients. The APP works as part of the patient care team in collaboration with a physician and the medical leadership team.

The APP is responsible for compliance with government regulations related to healthcare provision and participates in the organization’s Continuous Quality Improvement activities and Performance Management System. The position includes weekday shifts and after-hours call coverage by phone, with minimal hours. Applicants are asked to apply online and upload a cover letter and resume.

Requirements

• Two years of experience required.
• South Carolina license required.
• DEA/controlled substance authorization required.

Responsibilities
