About The Position

Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The provider will conduct examinations, review laboratory, referral, and imaging results to determine diagnoses, and accurately document visits using appropriate templates. The position requires accurate record-keeping within a 72-hour window and adherence to clinical care measures. The provider will offer comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting, encompassing health promotion, illness prevention, and a holistic healthcare model. This includes managing chronic diseases and acute illnesses, possessing broad knowledge in procedural skills, coordinating care with specialists, and utilizing community resources. The role advocates for healthcare access through structured evaluation, early intervention, and health promotion, consulting with supervising physicians or team members as needed. The provider will promote the Patient-Centered Health Home model of care, deliver excellent customer service, and behave courteously towards patients, families, and staff. Awareness of wait times and providing patient education materials tailored to health literacy are essential. The role involves promoting and participating in daily huddles, incorporating ancillary health team members in patient care, and setting/monitoring self-management plans. Responsibilities include assisting in the assurance of clinical procedures, maintaining up-to-date clinical protocols, and completing chart and peer reviews. The provider will also assist the patient care team with performance improvement, staff education, FHCN promotion, and performance evaluations. Participation in the rotation of medical, physician assistant students, and dental residents is expected, as is participation in Network Health Fairs and community events, and site and provider meetings. Meeting Network and credentialing expectations for licensure and employment is required. Adherence to the Attendance and Absenteeism Policy is mandatory, recognizing that regular attendance is an essential function. The ability to present to and work at any FHCN location is also required. Other duties as assigned.

Requirements

Completion of an accredited Nurse Practitioner, Certified Nurse Midwife, or Physician Assistant training program, with a minimum cumulative GPA of 2.5; or Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
Equivalent of extensive formal training as a Nurse Practitioner or Physician Assistant, including an understanding of the application of the theory and practices of the profession to the operation of the organization as part of a Master’s degree (or other applicable extended training program).
Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
Licensed in the state of California as a Nurse Practitioner, Certified Nurse Midwife and/or licensed in the state of California as a Physician Assistant.
Effectively communicate professional opinions and extrapolations of information collected, synthesized/analyzed.
Determine appropriate methods of communicating information through the use of tables, graphs, charts, and other visual forms.
Preparation and execution of presentations to large groups.
Regularly required to sit and use repetitive hand movements to type and grasp.
Frequently required to stand or walk.
Ability to lift and/or move up to 20 pounds.

Consult with supervising physician or other team members on cases that need further input.
Works within and helps to promote the Patient-Centered Health Home model of care.
Provide excellent customer service and care.
Consistently behave courteously when interacting with patients/ family members, support staff, and Network staff.
Remain aware of wait time during the daily workflow.
Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
Achieve patient status as measured by relevant clinical quality measures.
Promote and participate in daily huddles.
Incorporate the participation of ancillary health team members in managing the care of patients/families.
Set and monitor self-management plans.
Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
