Advanced Practice Provider - Telehealth
About The Position
The Advanced Practice Provider delivers timely virtual care to eligible patients following hospital discharge. The role addresses immediate clinical and care-coordination needs, supports safe transitions, and provides a clear handoff or bridge to primary care and other appropriate services. The APP sees patients for acute issues and chronic condition management, works within the program’s scope, and follows established protocols for escalation and follow-up. This is a 100% remote position with flexibility in working hours, required weekday and weekend availability, a closing-shift requirement, and no call requirements. Applicants may live in Indiana, Nevada, South Carolina, South Dakota, Tennessee, Texas, Virginia, West Virginia, or Wisconsin. The provider will be required to obtain Virginia and North Carolina licenses.
Requirements
- APP licensure and credentials appropriate for the assigned service area.
- A minimum of five years of clinical experience as an APP in family medicine, emergency medicine, or long-term/post-acute care, including management of medically complex adults and focused virtual assessments.
- Strong communication, organization, care-coordination, and independent decision-making skills.
- Must obtain Virginia and North Carolina licenses.
- Must be available to work weekdays and weekends and meet a closing-shift requirement.
Responsibilities
- Review discharge records and complete focused assessments, medication reconciliation, and time-limited care plans.
- Manage appropriate lower-risk diagnoses and chronic conditions within program scope.
- Recognize red flags and escalate to emergency, specialty, in-person, or higher-acuity care using established protocols.
- Coordinate referrals and services, including home health, wound care, social work, behavioral health, pharmacy, and durable medical equipment.
- Complete timely documentation, continuity-of-care paperwork, and clear handoffs for tests, referrals, and follow-up needs.
- Provide virtual care for acute issues and chronic condition management following hospital discharge.
