About The Position

Crossroads Health is a 501(c)(3) non-profit organization with facilities in Mentor, Painesville, and Cleveland, dedicated to providing comprehensive integrated behavioral and primary healthcare, early childhood services, and extended housing and recovery services to Northeast Ohio communities, regardless of ability to pay. Our mission is to offer hope, healing, and healthcare to all. We are seeking an Advanced Practice Provider (APP) to join our Primary Care Health Center team at locations in Mentor, Willoughby, and Euclid. The APP, who can be a Physician Assistant (PA), Nurse Practitioner (NP), or Advance Practice Registered Nurse (APRN), will work collaboratively with a physician under a standard collaborative care arrangement to deliver primary care services within a multidisciplinary health team. The role involves providing comprehensive, patient-centered care in an integrated healthcare setting, including individual services to patients assigned by Crossroads Health in the Integrated Healthcare Center.

Requirements

  • Master's Degree (minimum)
  • Licensed in the State of Ohio as a Physician Assistant, Nurse Practitioner, or Advanced Practice Registered Nurse
  • Medicare Provider Number
  • Controlled Substance Registration License in Ohio
  • Professional Liability Insurance.
  • Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable)
  • BLS/First Aid

Nice To Haves

  • Experience in a hospital or family practice setting as a primary care APP (one year preferred)

Responsibilities

  • Provide general comprehensive primary medical care services to individual patients across the life span in the office setting, including behavioral health assessment, medication evaluation and management, and prescription of medication in compliance with Ohio state regulations using a trauma-informed approach.
  • Provide diagnosis, screening, education, treatment, referrals, general medical procedures, and follow-up of services.
  • Provide medical coordination of ongoing treatment involved with the transfer or discharge planning of health center patients in various settings.
