Advanced Practice Provider - Palliative Care, Anderson
About The Position
Community Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Anderson, IN. The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need. This is a 1.0 FTE position, weekdays. This provider will work autonomously and collaboratively with our palliative care multi-disciplinary team at Community Hospital Anderson to deliver palliative and health care services to patients. The role focuses on comfort measures, end-of-life care, symptom management, and assisting with consults and new admissions. Flexibility to travel when needed is also a requirement.
Requirements
- 1-3 years’ Experience as a licensed Nurse Practitioner in Palliative Care
- Comfortable assessing patients independently
- Compassionate caregivers with excellent organizational, communication, and critical thinking skills
- Highly developed interpersonal and group dynamic skills
- Proven skills as a consultant/educator to friends, families, and loved ones
- Educational background consistent with requirements for licensure and certification within the state of Indiana
Responsibilities
- Deliver palliative and health care services to patients autonomously and collaboratively with the palliative care multi-disciplinary team.
- Focus on comfort measures of patients.
- Provide end-of-life care.
- Manage patient symptoms.
- Assist with consults and new admissions.
