About The Position

The Advanced Practice Provider (APP)-Palliative Care delivers comprehensive, patient-centered care to individuals living with a serious illness. The role focuses on assessment, management, and relief of symptoms and stress related to serious and life-limiting conditions, with the goal of improving quality of life for patients, caregivers, and families. The APP provides care to both new and established patients across multiple settings, including the home, skilled nursing facilities, assisted living, long-term care facilities, clinics, and hospitals. Working collaboratively with community providers and partners, Bluegrass Care Navigators’ physicians, and interdisciplinary team members, the APP follows established palliative care and symptom management protocols while exercising independent clinical judgment within scope of practice. This position plays a key role in developing, implementing, and communicating individualized plans of care and may contribute to clinical protocol development, education, and quality improvement initiatives related to palliative and end-of-life care.

Requirements

  • Graduate of an accredited Master’s or Doctoral program in Nursing or a Physician Assistant Program.
  • Current certification as an Advanced Practice Registered Nurse (ACNP, AGNP, FNP) or Physical Assistant.
  • Currently licensed as a Nurse Practitioner or Physician Assistant in the state of Kentucky.
  • Maintains current BLS certification.
  • Demonstrates clinical experience in hospice, palliative care, home health, medical, surgical or critical care settings.
  • Demonstrates in-depth, current clinical knowledge within area of specialty and the ability to translate professional and organizational goals into effective practice.
  • Skilled in supporting patients and families experiencing emotional stress; respectful of diverse lifestyles, values, and beliefs.
  • Commitment to the evidence-based practice, research utilization, and continuous professional development.
  • Ability to organize and utilize resources efficiently and effectively.
  • Strong interpersonal, communication, collaboration, and conflict-resolution skills.
  • Knowledge of healthcare systems and change management process.
  • Demonstrated ability to manage multiple priorities while effectively handling the emotional demands of palliative care practice.
  • Adheres to accepted professional standards, ethical principles, and organizational policies.
  • Strong clinical judgment, excellent observational skills, and the ability to provide timely, accurate written documentation.
  • Self-directed with the ability to work independently while functioning effectively within an interdisciplinary team.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Nice To Haves

  • Experience in Hospice and Palliative medicine preferred.
  • Strong skills in pain and symptom management.
  • I.V. therapy skills preferred.
