About The Position

HealthPartners is looking for a Certified Adult/Geriatric or Family Nurse Practitioner or Physician Assistant to join our Palliative Care team. Being part of our team means you will have an impact on the care that our patients receive every day. The APP works in collaboration with physicians and other medical staff to provide Palliative Care services to patients and their families. The APP engages in independent decision-making about health care needs and provides health care services to individuals and their families. The APP performs a comprehensive history, including chart review and information gathered from the family and performs physical and psychological health assessments. The APP provides direct patient care as well as consultative services to assure state of the art Palliative Care. The APP balances the workload and resources of the Palliative Care team. The APP is responsible for ensuring efficiency in the day to day activity of the department. This position will mentor, coach and support staff while carrying a patient case load. The APP will participate as an integral member of an interdisciplinary health care team and provide education to other health care professionals in various aspects of patient care. Requirements

  • Completion of a physician assistant or nurse practitioner (FNP or Adult/Gerontology) program
  • Certification via appropriate discipline
  • MN Licensure
  • Current BLS, ACLS certification
  • DEA registration

Nice To Haves

  • Three (3) or more years of experience in Palliative Care or related fields.
  • Minimum of 1-3 years as APP (NP or PA)
  • Palliative Care Certification
  • Strong understanding of Palliative Care
