The Advanced Practice Provider (APP) is a Physician Assistant-Certified or Certified Nurse Practitioner who delivers comprehensive, relationship-based care in an enhanced primary care clinic. The APP evaluates and treats acute concerns, manages chronic conditions, provides preventive care, and helps patients navigate the broader health care system. The role combines sound clinical judgment with accessible, coordinated, and highly personalized care for ProVia employees and eligible family members. Working as part of a collaborative clinic team, the APP develops evidence-informed care plans, communicates them in plain language, and follows important needs through resolution. The provider is expected to practice efficiently without making care feel rushed, recognize when consultation or a higher level of care is needed, and build the trust required for patients to stay engaged in their health. The clinic will be owned by ProVia and developed and operated by Branches Healthcare. The APP must protect the trust that makes this model work: clinical information is confidential and is not shared with the employer without the patient’s appropriate authorization, except as required by law. Patient Experience Commitment: Every patient should feel heard, respected, and confident in the plan. The APP is expected to listen without rushing, explain choices clearly, anticipate barriers, respond reliably, and take ownership of coordinating the next step until the loop is closed.
