About The Position

The Advanced Practice Provider (APP) is a Physician Assistant-Certified or Certified Nurse Practitioner who delivers comprehensive, relationship-based care in an enhanced primary care clinic. The APP evaluates and treats acute concerns, manages chronic conditions, provides preventive care, and helps patients navigate the broader health care system. The role combines sound clinical judgment with accessible, coordinated, and highly personalized care for ProVia employees and eligible family members. Working as part of a collaborative clinic team, the APP develops evidence-informed care plans, communicates them in plain language, and follows important needs through resolution. The provider is expected to practice efficiently without making care feel rushed, recognize when consultation or a higher level of care is needed, and build the trust required for patients to stay engaged in their health. The clinic will be owned by ProVia and developed and operated by Branches Healthcare. The APP must protect the trust that makes this model work: clinical information is confidential and is not shared with the employer without the patient’s appropriate authorization, except as required by law. Patient Experience Commitment: Every patient should feel heard, respected, and confident in the plan. The APP is expected to listen without rushing, explain choices clearly, anticipate barriers, respond reliably, and take ownership of coordinating the next step until the loop is closed.

Requirements

Graduate of an accredited Physician Assistant or advanced practice registered nursing program appropriate to the role.
Current, unrestricted Ohio licensure as a Physician Assistant or Certified Nurse Practitioner, current national certification, and prescriptive authority appropriate to the role and practice setting.
Current Basic Life Support certification required.
DEA registration and other credentials required when applicable to assigned duties.
Ability to complete credentialing, privileging, background checks, and competency validation.
Proficiency with electronic health records, secure messaging, telehealth, and clinical documentation, with excellent written, verbal, and patient-education skills.

Nice To Haves

Five or more years of APP experience preferred.
Primary care, family medicine, internal medicine, or multispecialty practice experience is strongly preferred.
Demonstrated ability to manage a broad patient panel, recognize clinical risk, interpret diagnostic information, coordinate complex care, and practice within scope and privileges.

Responsibilities

Build trusted relationships.
Create a welcoming, respectful environment; understand each patient’s goals, values, concerns, and preferences; and involve patients in decisions.
Listen and communicate clearly. Use open-ended questions, plain language, teach-back, and culturally responsive communication so patients understand their condition, options, and next steps.
Deliver unhurried attention. Balance access and efficiency with the time needed to address the patient’s priorities and identify concerns that may otherwise go unspoken.
Own follow-through. Set clear expectations, communicate results and changes promptly, and remain accountable for important clinical and coordination tasks through resolution.
Reduce barriers to care. Recognize scheduling, cost, transportation, health-literacy, medication, caregiver, and other barriers and work with the team on practical alternatives.
Protect trust and confidentiality. Maintain clear boundaries between clinical care and employment functions and safeguard protected health information in every setting and communication channel.
Assess and diagnose. Obtain histories, perform examinations, develop differential diagnoses, and evaluate common acute, chronic, and preventive health needs within the provider’s education, licensure, privileges, and demonstrated competency.
Develop and execute care plans. Order and interpret appropriate tests; prescribe medications and treatments; perform approved procedures; and establish monitoring, education, and follow-up plans.
Manage chronic conditions. Provide longitudinal, evidence-informed care for common chronic illnesses, address comorbidities, monitor treatment response, and adjust plans with the patient and care team.
Deliver preventive care. Complete age- and risk-appropriate screenings, counseling, immunization planning, risk reduction, and wellness care.
Provide timely acute care. Evaluate same-day and episodic concerns, identify red flags, stabilize urgent conditions as appropriate, and direct patients to emergency or specialty care when needed.
Address whole-person needs. Recognize behavioral health, social, occupational, medication, and lifestyle factors that affect health and connect patients with appropriate resources.
Document accurately. Maintain timely, complete, and clinically meaningful electronic health records that support continuity, quality, compliance, and team communication.
Collaborate across the care team. Work closely with physicians, APPs, nurses, navigators, medical assistants, and front-office staff through huddles, case review, and real-time consultation.
Coordinate referrals and transitions. Select appropriate specialists and services, communicate the clinical question, review returned information, and integrate recommendations into the patient’s plan.
Close care gaps. Use outreach, registry information, and team workflows to address preventive, chronic, post-discharge, and high-risk follow-up needs.
Support access between visits. Respond to clinical messages, results, medication questions, and telehealth needs within established service and urgency standards.
Participate in shared call. Provide assigned call coverage within the provider rotation, respond according to clinic protocols, document significant encounters, and coordinate appropriate follow-up.
Consult and escalate appropriately. Seek physician or specialty input for complex, uncertain, high-risk, or out-of-scope concerns and activate emergency processes when indicated.
Support clinic readiness. Participate in peer review, training, emergency preparedness, policy development, and responsible selection of tests, treatments, referrals, and sites of care.
