About The Position

Franciscan Physician Network is seeking a Diabetes and Endocrinology Advanced Practice Provider to join their group at Franciscan Health Diabetes & Endocrinology Specialists in Lafayette, Indiana. The practice offers a team-oriented, family-like culture that empowers providers to focus on patient care. Teamwork, empathy, and a positive personality are essential. A full support staff is available to assist providers. The ideal candidate will have past or present endocrinology experience, a strong focus on clinical excellence and patient satisfaction, and exceptional interpersonal skills. The provider will work collaboratively with physicians to coordinate and deliver high-quality patient care and is expected to develop strong ties with the community and existing medical staff.

Requirements

Master's Degree
Physician Assistant Certified (PA-C) or Nurse Practitioner (NP) or Nationally Recognized Certification
Basic Life Support Program (BLS) - American Heart Association

Responsibilities

Coordinate and deliver the highest quality of patient care in collaboration with physicians.
Provide care for patients with common diagnoses including Type I Diabetes, Type II Diabetes, Insulin pumps, Continuous Glucose Monitoring, Primary Hyperparathyroidism, Hyperthyroidism, Hypothyroidism, Hypogonadism, Polycystic Ovarian Syndrome, and Pituitary-Adrenal Disorders.
Manage an average outpatient volume of 12-21 patients per day.
Potentially participate in outreach to surrounding locations.
